If you have a standard faceplate, its dead easy, just remove the screws, ease out the front plate, leaving the wires connected, slip in the vDSL plate, replace front plate with longer screws, provided with the vDSL plate and that's it.

If you have a filtered faceplate with extensions then it is a bit more complicated. First you have to get a standard faceplate, remove the wires from the filtered plate and fix them to the standard plate, being careful to replace the wires on the correct terminals. When you have done that, the procedure is the same as described before.

Thinking about it a bit more, I'd recommend contacting Sky first. I seem to recall that BT "promise" that upstream sync will be >22.5% of the downstream sync and I also suspect your upstream is outside the figure you were quoted on purchase. It's pure guesswork at this stage what the root cause is: could be noise, could be crosstalk, could be the wrong profile etc. so you'd be potentially wasting time and money fixing something blind.

I would have thought Sky would get Openreach to investigate. At the very least you might get DLM kicked off again which may well bring your US/DS into alignment. If they do pass it to Openreach and you get an engineer visit (probable) then if you ask nicely he/she may well fit a new VDSL faceplate for free.
