It's simple really,

..........................Phone - Splitter - Phone & Sky
BT Socket - Filter -
...........................ADSL - Router - PC


I still recommend a filtered faceplate and also connecting the router directly into the BT socket with an ethernet cable to the PC

No. A low quality extension will pick up interference at different times. Although it may seem that nothing is changing, in the environment of a household conditions are constantly changing. Switching on lights, microwaves, washing machines, even the cat curling up near the extension may have an effect. Taking this out of the equation would solve a lot of your problems.

And the 56k modem (which isn't listed above) must be connected to the phone side of the filter, not the router side.

An ADSL filter has two outlets: the one marked phone is filtered, the one marked ADSL is straight through i.e. unfiltered. The router must have an unfiltered connection whereas everything else must be filtered.
